- Official transcripts from each of the high schools that you have
attended
- A report of your ACT or SAT scores
- A statement describing your interest in science and math, your interest in teaching, your intended academic major, and what
you hope to do after you graduate from college
- A list of your achievements in science or mathematics including any math or science fair projects that you have created, any
science or math competitions that you have entered and the outcomes of those, any awards that you have received in science
or math, and any other activities related to science or math in which you have participated that would document your interest
and accomplishments
- A list of your achievements in teaching, any clubs or activities in which you have participated that are related to teaching,
or any other accomplishments that would document your interest in teaching
- A list and brief descriptions of your leadership and community service contributions in school, community, church, or organizations
- Your name, mailing address, phone number, and email address
This information should be typed or computer printed and will be evaluated in part based on the quality of the writing and
the accuracy of spelling and grammar.
