Problem Report Form
Faculty or Staff who are experiencing difficulty with audio-visual equipment (see below) should report their problems to the Office of Audio Visual Services using the following form and our staff will work to resolve the problem as soon as possible.
Problems with video conferences or distance education communications should be reported to the Audio Visual Services staff member on duty by going to room 1012 CPC or by dialing ext. 20693.
Problems with computer systems or projector carts should be reported to the Office of Technology Support Services.
